Client forms and information

We understand how hard it is to even think about coming to therapy. We've been there. We are a diverse organization with a mutual philosophy of helping you become whatever God ordained you to be. You bring strengths to therapy: we bring our compassion, knowledge and expertise. We truly care about our clients, and strive to always give them our best by using our therapeutic skills, our experience and our spiritual world view to help you achieve your goals.

Payments and Insurance

We want therapy to be affordable and appropriate to your needs. We are considered an out-of-network provider with most insurances. We can accept cash, check, medical/health savings accounts (HSAs) or credit cards for payment. We also can provide you with a superbill you can submit to your insurance company for partial reimbursement if you have mental health out-of-network benefits. Please verify with them directly to learn their process.

 

Why don't we accept insurance? While we recognize that insurance copays may be more affordable than private pay, submitting your claims to insurance companies leaves both you and us bound by their specifications on treatment and billing, which may not always be the most suitable for your specific situation. In addition, your personal information is not protected in the same way by insurance companies, and any diagnosis (which must be assigned in order to submit a claim) becomes part of your permanent medical record. We believe that we can best serve your needs and protect your privacy by remaining a private pay practice. Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

At your request, we will gladly provide clients of Sanctuary Christian Counseling with a “Good Faith Estimate” to help them estimate the expected charges they may be billed for receiving certain health care items and services. Please contact your therapist to request this form.
